I'm pretty sure my problem was not wiring, but a firmware issue. I jammed up the first extruder pretty good (was able to unclog later) and started using the second one. With multiple versions of Repetier I ran into the problem - so I don't think it has anything to do with the version ...

I had a similar problem - it seems to behave badly shaking its way to the top and ignoring the endstops. I purchased another GT2560 and replacement A4988 (stepper motor boards), but the problem remained. It didn't do it all the time, but it sometimes took 10-20 power cycles to get it ...

I got the nozzle cleaned out and got some test prints. I'm using PLA at 190 and have a glass bed set to 60, but I think that the hotend is way too hot... The model I'm printing is still liquid 10+ layers down from the printhead and is getting deformed as the new material sticks to it ...

After I changed the polarity, the motor was going in the right direction. I did try the tension on the extruder drive and played around with the temps, then I disconnected the tubes and tried pushing by hand.
