MK8 extruder feed issue

I have a Prusa I3 Mendel Reprap from Geetech built from kit, with 1 extruder, .4 mm oriface and 1.75 PLA and ABS filament.
I am have trouble with the filament feed, see the attached photo. The filament will not load correctly, it misses the hole in the threaded rod after going through the pinch rollers. I have to remove the fan and heat sink to load the filament, and sometimes when it retracts it wont go back in the threaded rod and the stepper grinds. I have the same trouble with both PLA and ABS filament.
Shouldn't there be a chamfer or countersink on the hole in the threaded rod so it feeds into the hole better? or am I missing a piece to guide the filament into this hole?
Sorry if this question has already been asked, but when I try to search on 'extruder' in this forum it says it is too common a word and will not search.

In fact there is a chamfer:
