Round Heated Bed incorrectly wired?

Can someone confirm this heated bed is wired correctly? It does not light the LED's and only 1/2 the board gets warm? I think terminals 2 and 3 need to be tied together for 12v? I can't find any pics or other information online. Any help would be appreciated.

Yes ,you're right .you should tie 2 and 3 together for 12V;
and if you use 24V,just tie 2and 3 together and it will be done.
